At Vitality Health Services, we provide compassionate, community-based support to help Minnesotans live with stability, dignity, and independence. Our services are designed for adults on Medical Assistance (MA) who are navigating mental health challenges, housing instability, or both.

Housing Stabilization Services:

  • Help you plan for, find, and move into stable housing.

  • Help you keep housing after moving in.

  • Provide assistance finding financial resources.

  • Help you communicate with landlords.

Adult Rehabilitative Mental Health Services

Adult Rehabilitative Mental Health Services (ARMHS) are one-on-one, in-person services that help adults with mental health conditions build the skills they need to live more independently

Whether you need support finding stable housing, managing daily tasks, or building skills to live more independently, Vitality Health Services is here to help. We serve adults on Medical Assistance living with mental illness, substance use challenges, disabilities, or age-related needs.
